Welcome To Abundance Health, LLC

We are committed to providing our clients with quality private duty nursing, registered nurse delegation services, and illuminating the healthcare experience as a whole for our clients.

My name is Tamica Gant, and I am the CEO and founder of Abundance Health, LLC. I am a registered nurse with 10 plus years of medical experience. I received my bachelor's degree in science of nursing from Seattle University.

Is to provide safe and efficient nursing care by incorporating evidence based practice. Our staff brings light and radiates the environments of our clients.

We assist other individuals with providing quality nursing care. As a teen, I would aid in my grandmother’s healthcare in the convenience of her home before she passed away. I witnessed how she was placed in a nursing home for a limited time due to not being able to have anyone at home to provide nursing care and rehabilitation services. My grandmother desired to be in the comfort of her own home and be surrounded by loved ones while receiving nursing care, and that was not an option for her. Once I entered the healthcare arena, I witnessed the need for quality private duty nursing, the challenge for patients to have the freedom of choice on where they receive their nursing care, and the demand for caregivers to be educated to provide care that impacts the clients in a positive way. Witnessing the needs of other individuals, inspired me to create a nursing company that will reshape nursing care and fill in the gaps of many individuals seeking private duty nursing and registered nurse delegation services.

Private duty nursing and registered nurse delegation services for our clients. We provide nursing care and nursing tasks for clients outside of the medical institutional settings. With providing these services, we are able to teach families and caregivers how to effectively provide care for clients, prevent caregivers from experiencing burnout by educating them how to efficiently provide safe care to the clients directly. We also provide comprehensive assessments, caregiver credential reviews, caregiver trainings, care plan reviews, diabetes management, wound and ostomy care, and quarterly assessment reviews.

Private duty nursing (PDN) is a program that offers in- home nursing care to clients who would be served in a medical institute. Private duty nursing is an alternative to institutional care. Private duty nursing provides community-based alternatives to institutional care for clients who have challenging medical needs (tracheostomies, ventilators, feeding tubes, continuous IV infusions), and require skilled nursing care on a continuous basis.

Registered nurse delegation (RND) is a program that is under the Washington state law. This program allows long term care workers (nursing assistant registered, home care aide certified, and nursing assistant certified) to provide certain tasks in certain settings that are delegated by the nurse. The tasks include, but are not limited to administering certain medications, blood glucose testing, ostomy care, clean dressing changes, gastrostomy (G-tube) feedings, bladder irrigations, clean suctioning for oral tracheostomies, catheter care, etc. 

Our staff will provide training, education, and supervision for the long term care workers with the nursing task that have been delegated to them, as well as assess the patients physically based on the plan of care that has been implemented, and evaluate the client’s conditions, and progress.

Step 2: Contact your nearest Home and Community Service Office with this link: https://www.washingtonconnection.org/home/, or if you have a case manager, speak with your case manager and request an intake. Inform the staff or your case manager that you would like to be assessed for PDN and/or RND services. Upon them determining eligibility, the nurse or case manager will determine what services you’re eligible for. You would let them know that you are interested in using Abundance Health, LLC’s services as a PDN and or RND provider, please provide them with the information that is listed below:

Step 3: Once eligibility is confirmed with your case manager (if you’re Medicaid eligible), we will schedule and conduct a phone or zoom interview to assess specific healthcare needs, and have the clients answer intake questions. It is our goal to fully understand your health condition and your needs. During the intake, we will gather information to better assist you. 

Step 4: Upon choosing Abundance Health, LLC as a provider, we will conduct an in person assessment, view the plan of care and medical orders that was established by the provider, assess the client’s healthcare regimen, and place of living.

Step 5: We will begin the process of PDN or RND. Throughout the process of providing nursing services, we will reevaluate the client’s conditions often to be sure the client gets the best care to promote health & wellness.
